Appointments:
We offer telehealth and in person appointments
To remain an active patient, you must attend at least 1 appointment with a provider quarterly. If 90 days have passed with no contact, or if you have missed 3 or more appointments, we may initiate the discharge process.
Clinical policies:
We obey Colorado law, meaning we cannot and do not prescribe medications for patients who are not yet patients and will not prescribe prior to initial evaluation.
We perform random urine drug screens on all patients being prescribed or considered for controlled substances. We also perform random drug testing should a patient’s status be concerning for risk of a substance use disorder and appear to clinically compromise the patient’s safety, or our safety as prescribing providers. Patient safety is our priority.
We will not prescribe stimulants at the first visit until confirmation of diagnosis and medical necessity. We routinely refer patients to get further neuropsychological testing to thoroughly and properly identify frontal executive functioning prior to prescribing stimulants. Any patient on a stimulant will be routinely monitored and be required to be seen in person 3-4 times a year at minimum.
Benzodiazepines are rarely prescribed at Well Nourished LLC. Any patient on a benzodiazepine will be routinely monitored and be required to be seen in person 3-4 times a year at minimum.
Good Faith Estimate
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees. Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your healthcare provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate. Prescription Refills and Prior Authorization
We kindly request 72 hours (3 days) notice for all Prescription Refills.
If your pharmacy does not have your medication in stock, please call your nearest local pharmacies to find a pharmacy that does. We would be happy to send over a new prescription.
Prior Authorizations can sometimes take up to 3-4 days to complete. We do our best to get these completed within the day but with the volume of patients, it may take longer. We kindly request your patience with this process.
Medication Dosage Changes/New Prescriptions
All dosage changes and prescriptions of new medications need to be done during a follow up appointment. These changes cannot be made over the phone or via email. If you are an existing patient and should you need a medication dosage or a prescription for a new medication, please call the office number at 720-513-1215 to schedule a follow up with your provider.
